Organization Information

Organizations must either have current 501(c)3 status or utilize a fiscal agent/sponsor with current 501(c)3 status to qualify for grants from the Takoma Foundation. If your organization has 501(c)3 status, please attach a PDF of your IRS Letter of Determination here. Otherwise, provide information on your organization’s financial structure and fiscal agent in the next section.
For those organizations without 501c(3) status and can qualify via a fiscal agent/sponsor with 501c(3) status, please provide the name of your fiscal agent and explain how your group operates financially.

Supporting Details

What will this project accomplish?
Describe who will perform the program’s activities, where will they take place and how will they be carried out? Are there other organizations or individuals who are participating in or supporting the activity?
Identify who will benefit from the project and how. How does this activity promote and facilitate a socially, environmentally, economically, and culturally healthy Takoma Park community? If the project does not take place within the greater Takoma Park area, please be specific about how it will benefit the Takoma Park Community.
Provide a schedule for major tasks and accomplishments. The Takoma Foundation can only fund projects that can be completed within 12 months.
Provide a budget (including any support you expect to receive from other sources) and, if available, an organizational budget. For the purpose of this application, support may be financial or in-kind (such as volunteer hours that community members have pledged to the project). Also, describe how funding from Takoma Foundation will impact the project.
How will you measure the success of the project? Describe how results will be reported to the Takoma Foundation.
